Job Description Home Health RN Location: Brattleboro, Vermont Job Type: 26 Weeks Contract Day shift - 8:00am - 4:30pm, Monday - Friday w/rotating on-call and weekends Guaranteed Weekly Hours: 40 Summary The Home Care RN uses clinical nursing knowledge, physical assessment, teaching, and procedural skills to deliver high-quality patient care in the patient's place of residence. The RN contributes to the development of the home health plan of care. The RN delivers patient care directed by the physician as established in the home health plan of care that is consistent with clinical best practices and results in high quality, improved outcomes, and exceptional patient experience. Responsibilities: Develop a plan of care under the direction of the client's physician and in collaboration with the client, their family, and/or the primary caregivers upon admission the needs of individuals and families within their homes and communities. Coordinates the client's care plan through interdisciplinary communication and case conferences, as appropriate. Advises other disciplines of discharges in a timely basis. Provides ongoing assessment of the client's clinical, functional, and emotional status to identify problems and/or progress toward desired goals. Prepares home health clinical notes and updates the case manager and primary physician when necessary. Completes, maintains, and submits accurate and relevant hospice clinical notes regarding the patient's condition and care given. Records pain/symptoms management changes /outcomes as appropriate. Requirements: Must have good nursing experience. Must have at least a year of Home Health experience. Must have Vermont RN Licensure, Valid driver's license.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
