Summary:
The Registered Nurse Case Manager is a member of the Medicaid Community Health Team (CHT) The Registered Nurse Case Manager participates in the Spoke program by providing comprehensive and coordinated services for patients undergoing Medication Assisted Treatment Services (MAT) specifically buprenorphine treatment.The primary focus of the MedicaidCHTis to assure that clients receive coordinated care that addresses all the aspects of Home Health Services.
Dept. Scope of Service:The Medicaid Community Health Team (CHT) is a multi-disciplinary team designed to coordinate access to high-quality, cost-effective and comprehensive Medication Assisted Treatment Services.The goal is to ensure that clients receive coordinated care that addresses all aspects of Home Health Services.The MedicaidCHTpartners with theBrattleboroMemorialHospital(theCHTAdministrative Entity), the Brattleboro Hospital Service Area buprenorphine prescribers, The Brattleboro Retreat, and Habit Opco. MedicaidCHTservices are available to all patients without eligibility requirements, prior authorizations, referrals or co-payments.Medicaid funds the MedicaidCHTas a shared resource.
Qualifications:
Education
State ofVermontRegistered Nurse
One year psychiatric/substance abuse nursing experience or two years general nursing experience
Experience
Minimum of one year of experience in psychiatric/substance abuse nursing.Excellent interpersonal or communication skills.Knowledge of behavioral aspects of chronic health and mental health issues.Strong patient advocacy skills and knowledge of community resources.Knowledge of State and Federal in Microsoft Office Applications.
Licensure/Certification
State ofVermont Registered Nurselicense.
Physical & Mental Efforts
Strong written and verbal communication skills.
Must demonstrate the ability to consistently exercise discretion and judgment regarding counseling of referred patients, appropriate to the scope of practice and licensure.
Maintains confidentiality.
May require bending, stooping and stretching.
May require the ability to reach or work at floor level.
May require lifting/carrying items up to 50 pounds.
Requires hand-eye coordination and manual dexterity.
Requires use of office equipment such as computer, etc.
Requires corrected vision/hearing to normal range.
May require working irregular hours.
Must demonstrate excellent organizational skills and ability to work independently.

Requires managing multiple priorities.
Requires working with interruptions.
